Community Health Worker

Description:
Community Health Worker LPN.
Experience in behavioral health preferred.
Assists the primary care provider in recognizing, treating and managing mental health and psychosocial issues and acts as a contributing member to the interdisciplinary team.
Conducts outreach visits to the patient as needed to ensure treatment compliance of the integrated treatment plan and making recommendations to the team when needed.
Initiates follow-up to ascertain how clients are doing and to determine if any changes in treatment approaches are indicated.
Assist patients in getting to/from appointments in the Integrated Health Home including arranging for transportation and/or transporting the patient via the Integrated Health Home Vehicle.
Monitors and coordinates the delivery of health services for clients as related to behavioral health care, including linking with other treatment providers not only within the primary care setting but, with the clients.
Requirements:
Experience in Behavioral Health preferred Current NJ LPN licensure Completion of Clinical Experience in NLN accredited school preferred BLS required Experience with community health preferred Ease in working with both inpatient and in-community environments a must Ability to work collaboratively with other disciplines.
Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
