We currently have an opportunity for a Case Manager/RN to work in our Case Management Department.
PER DIEM STATUS Description:
The Case Manager is a registered nurse who will coordinate the care of a group of patients across the continuum of care.
The Case Manager is responsible for optimizing patient outcomes efficiently while reducing the length-of-stay and also facilitating the proper use of resources.
PER DIEM STATUS Requirements:

RN license - BSN preferred - Minimum of five (5) years acute nursing experience - Previous supervisory/management experience helpful - Previous Case Management experience preferred.
Estimated Salary: $20 to $28 per hour based on qualifications.
