Constellation Health is looking for a compassionate and empathetic Licensed Independent Clinical Social Worker to work with our hospice patients.
This role will involve providing care in the patients' homes and as well as in Skilled Nursing Facilities.
Schedule:
Monday-Friday 24 to 32 hours per week with some flexibilities Territory:
Metro Lakewood, New Jersey This Is You:
LICSW in the state of New Jersey with at least 1 year of clinical experience; Hospice, Acute Care, and/or Bereavement Counseling preferred Have reliable transportation and willing to commute within a designated territory to provide the best in home patient care.
Together, we:
Provide Clinical Social Work to the Hospice populations.
Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, neglect or abuse and plan intervention based on evaluation findings Educate patients and caregivers on disease processes, medications, plan of care and individualized treatment plan Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
Submits all clinical documentation, time records and other information in the required time frame Perks of Constellation:
